The doctors consulting at ANMH are committed to supporting the long-term health and wellbeing of our local community. For patients living with a chronic medical condition, the doctors consulting at Altona North Medical Hub, together with onsite and external allied health practitioners, can work with you to create a personalised care plan that supports your ongoing health needs.

Care is focused on coordination, regular review and practical strategies that aim to support day-to-day management and overall quality of life.

What is a chronic condition?

Chronic conditions are long-term health issues that have lasted six months or more. Some common examples include:

  • Asthma
  • Cancer
  • Cardiovascular disease
  • Diabetes
  • Musculoskeletal conditions
  • Stroke

Patients seeking support for chronic disease in Altona or chronic disease care in Melbourne’s western suburbs are encouraged to discuss their individual needs with their GP.

What is a GP management plan (GPMP)?

A GPMP is a structured care plan developed in partnership with your GP. It outlines your specific health needs, the services your GP will provide, and steps you can take to better manage your condition. This plan helps ensure your care is coordinated and tailored to your goals.

Team care arrangements (TCA)

If your condition requires input from multiple health professionals, your GP may also put in place a Team Care Arrangement. This involves your GP working closely with at least two other healthcare providers—such as physiotherapists, dietitians or specialists—to ensure you receive the right support across all areas of your care. Having a TCA from your GP often results in heavily subsidised or bulk-billed sessions with your allied health practitioners.

Is there a cost?

Eligible patients can access care plans at little or no cost. To find out if you qualify, simply book an appointment with your GP. We’re here to support your journey to better health, every step of the way.
Your care plan will be reviewed regularly by your GP to make sure it stays aligned with your needs. This gives you and your doctor the chance to review your progress and update your goals as needed. Our practice nurse is also available to help monitor your condition and support you between appointments.

Supporting long-term health goals

Living with a chronic condition can affect many aspects of daily life. GPs consulting at ANMH aim to support patients in setting realistic, achievable health goals that align with their individual needs.

This may include:

  • Monitoring and reviewing medications
  • Discussing lifestyle factors such as physical activity and nutrition
  • Supporting self-management strategies
  • Coordinating referrals where appropriate

Patients seeking support for chronic disease management in Altona are encouraged to book regular reviews to help maintain continuity and adjust care as required.

Access to allied health services

Altona North Medical Hub offers access to a range of onsite and external allied health services. Where appropriate, your GP may recommend involving allied health practitioners as part of your care.

This integrated approach can support patients managing chronic disease in Melbourne, helping reduce the need to attend multiple locations and supporting coordinated care within a single healthcare setting.

Frequently asked questions
+
How do I know if I’m eligible for a GP Management Plan?
Eligibility depends on whether you have a chronic medical condition that requires ongoing management. Your GP will assess your situation during an appointment and advise if a GPMP is appropriate.

+
How often are care plans reviewed?
Care plans are typically reviewed at least every six months, or earlier if your health needs change. Reviews help ensure the plan remains relevant and effective.

+
Do I need to see the same GP for chronic disease management?
Where possible, seeing the same GP may support continuity of care. However, other doctors consulting at the clinic can also access your records and assist when needed.

+
Can care plans help with allied health costs?
For eligible patients, a Team Care Arrangement may allow access to Medicare-subsidised allied health sessions. Your GP can explain what may be available to you.

+
Can lifestyle changes be included in my care plan?
Yes. Care plans often include practical lifestyle strategies tailored to your condition, preferences and capacity, alongside medical management.

Worst customer service ever experienced I can’t believe how rude this receptionist lady was on the phone absolutely 0 respect she should get fired!! For managers referral this phone call was on 21/01/2025. She was being aggressive on the phone when I asked her if I can come in and then she hung up in my face. I then kept trying to call back to book an appointment as this is my local clinic and I was never answered too.

My family and I have been regulars at Altona Medical Clinic for over 20 years. Generally the service is good, but over the last 12 months trying to get an appointment is woeful. Not only must you ring at 8am for an urgent appointment, you are slotted against changing Drs. I tried today at 8am to be told no appointment for 2 days. Sadly that is pushing us out the door to alternative options. Don’t even bother if your a new patient!

Terrible experience on 20/11 with the female admin staff at kidney west. She’s rude, unhelpful, and completely disorganized. Patients deserve better care and respect, especially in a medical setting.

The admin was rude toward the person who was on the phone to have an inquiry. The admin turned around and said the person who was on the phone was having attitude. Could the admin be more patient and listen to inquire before her negative conclusion.

I haven't been a patient since August 2023 & receive a random missed call, no message left. When I contact them they said i'd seen a doctor the day earlier, was this about payment? No, I wasn't there yesterday & why has someone accessed my patient details to call me? Reception had zero idea as to why I'd been called as there were no notes on my file. I now have serious concerns about my confidential information lodged with them. After 2 hours of awaiting a reply regarding this I have decided to post this review. Beware, as they seem to be unable to explain how or why my information was used & the person on reception seemed to not even care that this was of concern.

19 years I’ve been coming to this practice with our whole family but today is my last appointment ever. On my last visit the doctor was so lacking in interest in me and the reason I was there. Today I’ve sat for 1.5 hours in the car waiting to see doctor outside because I had a fever. After an hour I phoned to be told told I’m next and doctor running an hour behind. Multiple phone calls over next 30 mins and I am repeatedly told I’m next. I’m so sick I just want to go home and crawl into bed. This is cruel and unusual punishment. I’m giving up and going home. To prove how money hungry they are - after 1 hour 40 mins waiting in car - when I phone to say I was cancelling and going to another clinic THEN the doctor can phone me within 2 mins. Review your practices. You’re a disgrace of a clinic. You won’t ever see me again
